Access Secure Forms

1. Sign in to your My Services page.

2. Click the Secure Forms link. 

   

Secure Forms Dashboard

In the Secure Forms dashboard, you can add and manage existing forms and themes and review form submissions:

• Manage Forms

• Manage Themes

• Review Submissions 

  

Manage Forms

In the Manage Forms section, you build new forms and edit existing forms. 

• Create a secure form

• Edit a secure form

Manage Themes

In the Manage Themes section, you can create and modify existing themes. Themes are templates that surround your form fields, such as the header, body and footer.

Review Submissions

The Review Submissions section is where you review the forms that were submitted. The forms are listed by name. After clicking the form name, the form fields are listed along with icons that allow you to view the submissions in PDF or Excel format.
